Notice of Infringement – Claim

- Identification of the infringing material and its location (please include the URL);
- Contact information of the complaining party, including name, address, email, phone, and fax number;
- A statement that the complaining party has a good faith belief that the use of the material is unauthorized;

Title 17 USC §512(f) imposes penalties for misrepresentation in a notification of infringement.

Counter Notification – Restoration of Material

- A statement, under penalty of perjury, that the removal was a mistake;
- Your name, address, telephone number, and consent to jurisdiction of the federal district court.
